Citations are only as good as the retrieved source metadata. Recent ingestion work adds chunk indexes, headings, offsets, source titles, and URLs to improve provenance.
- If an answer cites the wrong source, check the agent's assigned source list.
- If an answer is vague, inspect whether the source extracted meaningful text.
- If an answer should never vary, move it into a direct response.
